Subtalar Posterior Recess

recessus posterior articulationis subtalaris

The posterior subtalar joint recess is the synovial extension of the posterior subtalar joint between the posterior calcaneal facet and the posterior talar facet, accessible via the posterolateral ankle arthroscopic portal.

Anatomical Data

Boundaries & Contents

LocationBack of the posterior subtalar joint, between the posterior talus and the calcaneus
Boundaries
  • Posterior process of the talus (above)
  • Upper surface of the calcaneus behind the posterior facet (below)
  • Posterior talocalcaneal ligament and capsule (behind)
  • Flexor hallucis longus tendon nearby (medial)
Contents
  • Synovial fluid
Biomechanics

Function & Actions

A pocket of the posterior subtalar joint that collects fluid and loose bodies. In some people it communicates with the ankle joint, and it is reached through the posterior hindfoot arthroscopy portals.

Clinical Relevance

Clinical Notes

The posterior subtalar recess contains loose bodies and synovitis in subtalar joint arthritis. Posterior subtalar arthroscopy (hindfoot arthroscopy) accesses this recess for subtalar fusion preparation and loose body removal. Subtalar joint effusion filling this posterior recess is seen in inflammatory arthropathy and post-traumatic subtalar arthritis.

Pathology

Common Injuries & Conditions

Subtalar Joint Synovitis

Inflammatory synovitis in the posterior subtalar recess producing hindfoot pain with uneven ground walking, diagnosed by ultrasound-guided injection response and managed with arthroscopic synovectomy or subtalar fusion.
