Ankle Anterior Recess

recessus anterior articulationis talocruralis

The anterior recess of the ankle joint lies between the anterior inferior tibiofibular joint capsule and the anterior talar dome, at the level of the ankle mortise anterior to the tibiotalar articulation. It is the site of anterior ankle impingement synovitis and osteophyte formation, accessible from the anterior ankle arthroscopic portal. The capsule bulges anteriorly with ankle effusion into this recess.

Biomechanics

Function & Actions

Lets the anterior capsule fold up during dorsiflexion and is where ankle effusion first shows on ultrasound. Osteophytes and scarred synovium here cause anterior ankle impingement.

Clinical Relevance

Clinical Notes

The anterior ankle recess is the primary target for ankle joint aspiration and corticosteroid injection, performed under ultrasound guidance through the anterior approach between the tibialis anterior and extensor hallucis longus tendons. Anterior ankle impingement involves osteophyte formation at the anterior talar neck and distal tibial margin encroaching on this recess, causing pain at end-range dorsiflexion from impingement of the capsule and synovium. Arthroscopic debridement of anterior ankle osteophytes and synovitis from this recess is one of the most commonly performed ankle arthroscopy procedures.

Pathology

Common Injuries & Conditions

Anterior Ankle Impingement

Repetitive forced dorsiflexion in soccer players and dancers produces osteophyte formation at the anterior talar neck and distal tibia that encroaches on the anterior recess, causing anterior ankle pain at end-range dorsiflexion, managed with physiotherapy, injection, and arthroscopic osteophyte excision.

Ankle Effusion

Accumulation of synovial fluid in the anterior ankle recess from sprain, fracture, or arthritis produces a soft fluctuant swelling anterior to the mortise, best assessed by ultrasound and aspirated under ultrasound guidance through the anterior approach.
