Flexor Hallucis Brevis Medial Head

caput mediale m. flexoris hallucis brevis

The medial head of FHB inserts via the medial sesamoid and is primarily responsible for the medial sesamoid position during push-off. The medial head is the component addressed in hallux valgus surgery when the medial sesamoid is displaced.

Nerve: Medial plantar nerve (S1, S2) Blood Supply: Medial plantar artery Region: Hand & Wrist
Anatomical Data

Origin, Insertion & Supply

OriginCuboid and medial cuneiform plantar surface
InsertionMedial base of the hallux proximal phalanx via the medial sesamoid
Nerve SupplyMedial plantar nerve (S1, S2)
Blood SupplyMedial plantar artery
Biomechanics

Function & Actions

ActionsFlexes the hallux MTP joint — medial contribution; Stabilises the medial sesamoid during push-off
Clinical Relevance

Clinical Notes

Medial sesamoid instability from medial FHB head laxity contributes to hallux valgus deformity progression. In hallux valgus surgery, the medial capsule and medial FHB head are tightened as part of the medial soft tissue repair.

Palpation

Palpated plantar-medial to the first MTP joint during hallux MTP flexion.
