Mandibular Second Premolar

dens premolaris secundus inferior

The mandibular second premolars sit between the first premolar and the first molar in the lower jaw (Universal 29 on the right and 20 on the left, FDI 45 and 35). They have one buccal cusp and either one or, often, two lingual cusps, giving a Y-shaped groove pattern, and a single root. They erupt at about 11-12 years.

Clinical Relevance

Clinical Notes

After the third molars, the lower second premolar is one of the teeth most often missing from birth, in which case the baby molar above it may be kept for many years. The mental foramen lies near its root tip, so lower lip numbness can follow surgery or infection here. Like the first premolar it can carry a dens evaginatus.

Pathology

Common Injuries & Conditions

Congenitally Missing Second Premolar

The tooth never forms, leaving a retained baby molar or a gap that may need an implant or orthodontic closure.

Dens Evaginatus

An extra pulp-containing tubercle on the biting surface that can fracture and lead to pulp infection.
