Wrist Dorsal Radiocarpal Space

The dorsal radiocarpal space is the dorsal recess of the radiocarpal joint, accessible via the 3-4 portal in wrist arthroscopy. It contains the dorsal ligamentous structures and is the site of dorsal capsular tears, scapholunate ligament dorsal component tears, and dorsal wrist ganglion roots.

Wrist arthroscopy via the 3-4 portal (between EPL and EDC tendons) directly accesses the dorsal radiocarpal space. Scapholunate instability involves the dorsal scapholunate ligament accessible in this space. Dorsal capsular rent is the origin of dorsal wrist ganglion, identifiable as a one-way valve at the scapholunate interval. Thermal shrinkage of the dorsal capsule for instability targets this space.
