The talonavicular joint is the medial component of the Chopart joint and the most mobile of the hindfoot-midfoot joints, allowing the hindfoot to position the forefoot for terrain adaptation. It receives the talar head in its cup-like navicular socket, supported inferiorly by the spring ligament and dynamically by the posterior tibial tendon.
Talonavicular joint fusion alone eliminates 90 percent of hindfoot inversion-eversion, effectively fusing the hindfoot without subtalar or calcaneocuboid fusion. It is specifically used for isolated talonavicular arthrosis in rheumatoid arthritis. The spring ligament failure in tibialis posterior tendon dysfunction produces talar head medial and plantar tipping at this joint.
Midfoot joint degeneration managed with orthotics and talonavicular arthrodesis for refractory cases.
