The suprapatellar plica is the remnant of the embryonic partition between the suprapatellar pouch and the main knee joint cavity, present in 20-80% of knees as a transverse shelf superior to the patella. It may become thickened and symptomatic in plica syndrome.
Suprapatellar plica syndrome produces anterior knee pain with a palpable band superior to the patella that pops or clicks during knee flexion. The plica is identified arthroscopically as a transverse fold in the suprapatellar pouch and may cause articular cartilage damage on the medial femoral condyle. Arthroscopic plica resection is effective when conservative treatment fails.
Symptomatic hypertrophied suprapatellar plica producing anterior knee pain, a superior patellar snap with flexion, and potential medial condyle cartilage damage, treated by arthroscopic plica excision.
