The posterior subtalar joint recess is the synovial extension of the posterior subtalar joint between the posterior calcaneal facet and the posterior talar facet, accessible via the posterolateral ankle arthroscopic portal.

Clinical Notes

The posterior subtalar recess contains loose bodies and synovitis in subtalar joint arthritis. Posterior subtalar arthroscopy (hindfoot arthroscopy) accesses this recess for subtalar fusion preparation and loose body removal. Subtalar joint effusion filling this posterior recess is seen in inflammatory arthropathy and post-traumatic subtalar arthritis.

Inflammatory synovitis in the posterior subtalar recess producing hindfoot pain with uneven ground walking, diagnosed by ultrasound-guided injection response and managed with arthroscopic synovectomy or subtalar fusion.
