Right Paracolic Gutter

The right paracolic gutter is the peritoneal recess between the lateral border of the ascending colon and the right lateral abdominal wall, continuous superiorly with the right subhepatic space (Morison pouch) and inferiorly with the right iliac fossa and pelvis. It provides a direct route for peritoneal fluid to spread from the pelvis to the right subphrenic space and vice versa.

Clinical Notes

The right paracolic gutter is the most clinically significant paracolic gutter because it is wider and communicates more directly with the hepatorenal recess. Appendix rupture produces pus and faeculent fluid that tracks up the right paracolic gutter to the right subphrenic space, causing right subphrenic abscess. Peritoneal metastases (from ovarian, colonic, or gastric cancer) implant preferentially in the right paracolic gutter due to peritoneal fluid flow dynamics. Drainage catheters for right iliac fossa collections are placed to also drain the gutter.

Right Subphrenic Abscess from Ruptured Appendix via Paracolic Gutter

Appendix perforation with faeculent peritonitis produces pus that tracks up the right paracolic gutter to the right subphrenic space when the patient is upright, producing a right subphrenic abscess with right shoulder tip pain from diaphragmatic irritation managed by CT-guided percutaneous drainage.
