The retrohepatic space lies between the posterior liver surface and the anterior inferior vena cava, within the hepatorenal ligament attachments. It is a potential space for fluid accumulation in liver trauma and hepatic vein obstruction.

Clinical Notes

The retrohepatic space is accessed during right hepatic vein and IVC exposure in major hepatic surgery and liver transplantation. Retrohepatic haematoma from liver trauma or hepatic vein injury produces blood accumulation in this space compressing the IVC. The caudate lobe of the liver partially occupies this space and is approached via the retrohepatic corridor.

Retrohepatic Haematoma

Blood accumulation between the liver posterior surface and IVC from hepatic parenchymal injury or hepatic vein avulsion, producing IVC compression and haemodynamic instability requiring damage control hepatic packing.
