The radiohumeral joint is the articulation between the spherical capitellum and the concave radial head, located within the common elbow synovial joint. It participates in both elbow flexion-extension and forearm rotation, making it the most versatile of the three elbow articulations. Lateral epicondylalgia, plica syndrome, and osteochondritis dissecans of the capitellum all affect this joint.
Radial head arthroplasty for comminuted radial head fractures must restore the correct radial head height to maintain proper radiohumeral and distal radioulnar mechanics. Overlengthening produces radial capitellar overstuffing with stiffness and pain; shortening produces DRUJ instability and proximal radius migration.
