The radiocapitellar joint provides both elbow stability (resisting valgus forces alongside the MCL) and the proximal pivot of forearm rotation. The radial head acts as a valgus stress absorber — its excision in the presence of MCL deficiency produces valgus instability. Osteochondritis dissecans of the capitellum in adolescent overhead athletes produces the loose bodies and locking of the adolescent baseball pitcher's elbow.

Clinical Notes

Capitellar OCD (osteochondritis dissecans) from repetitive valgus compression in young overhead athletes produces lateral elbow pain and mechanical symptoms. Stable lesions are managed non-operatively; unstable or detached fragments require arthroscopic drilling or ORIF. Advanced OCD with subchondral collapse requires osteochondral allograft or autograft reconstruction.
