Lesser Sac Inferior Recess

The inferior recess of the lesser sac extends between the layers of the greater omentum toward the transverse colon, forming the inferior boundary of the omental bursa. It may accumulate fluid in lower pancreatic pseudocyst and mesenteric haematoma.

Clinical Relevance

Clinical Notes

The inferior lesser sac recess is the pathway for lesser sac collections to extend toward the pelvis in severe pancreatitis. Pancreatic walled-off necrosis may extend into this inferior recess producing a collection between the omental layers that is distinct from the main body of the lesser sac. Endoscopic transgastric drainage may not reach this inferior extension, requiring separate percutaneous access.
