The lateral thigh compartment is defined by the lateral intermuscular septum medially, the fascia lata laterally, and contains the vastus lateralis anteriorly. The tensor fascia lata and iliotibial band contribute to the lateral compartment fascia. This is the least commonly affected compartment in thigh crush injuries.
Lateral thigh compartment syndrome is rare but occurs in severe thigh crush injuries alongside anterior and posterior compartment involvement. Three-compartment fasciotomy is required for complete thigh decompression. The lateral femoral cutaneous nerve runs adjacent to the lateral compartment fascia and is at risk during fasciotomy. Haematoma from femoral shaft fracture most commonly accumulates in the anterior thigh compartment but may involve the lateral compartment.
