The intermetatarsal joints between adjacent metatarsal bases are small plane synovial joints that stabilise the transverse metatarsal arch. They are bound by the deep transverse metatarsal ligament distally and intermetatarsal ligaments at the bases. Metatarsal stress fractures and Morton neuroma both occur in the intermetatarsal spaces.

Clinical Notes

The deep transverse metatarsal ligament between the metatarsal heads is the structure through which Morton neuroma forms — the interdigital nerve is compressed between adjacent MTP joints and the ligament during toe extension, progressively forming perineural fibrosis.

Morton Neuroma Development

Interdigital nerve compression against the deep transverse metatarsal ligament during loading produces perineural fibrosis and the characteristic web space burning of Morton neuroma.
