The sciatic notch space at the hip refers to the combined greater and lesser sciatic notch region at the posterolateral pelvis, a critical neurovascular passage point between the pelvic interior and the gluteal/thigh regions. Multiple structures traverse this region including the sciatic nerve, superior and inferior gluteal vessels and nerves, pudendal nerve, and obturator internus.

Clinical Notes

The sciatic notch region is the target for piriformis injections, sciatic nerve blocks, and posterior hip joint injections. It is the site of deep gluteal syndrome pathology where the sciatic nerve may be compressed by various structures. Posterior hip arthroplasty approaches require careful navigation through this region. Haematoma collecting in the sciatic notch space compresses the sciatic nerve producing post-operative foot drop.
