Glenohumeral Inferior Capsular Space

The inferior glenohumeral capsular space (axillary pouch) is the most capacious part of the glenohumeral joint, accommodating the inferior glenohumeral ligament complex. It is reduced and fibrosed in adhesive capsulitis and is released in capsular release surgery.

Clinical Notes

The inferior capsular space is the last portion to release in arthroscopic frozen shoulder surgery — after anterior and posterior capsular release, the inferior capsule release restores external rotation in abduction. Inferior capsule tightness produces a positive anterior impingement sign from superior migration of the humeral head. The inferior capsule is tensioned in adduction and relaxed in abduction.

Fibrosis of the inferior glenohumeral capsular space producing loss of external rotation in abduction — the most functionally disabling aspect of adhesive capsulitis — released arthroscopically as the final step in capsular release.
