The humeroulnar joint is the principal hinge of the elbow, with the deep trochlear notch of the ulna wrapping around the humerus trochlea to provide inherent bony stability. The coronoid and olecranon stabilise against anterior and posterior dislocation respectively. The medial UCL stabilises this joint against the valgus forces of throwing.
Ulnohumeral joint arthritis from OA or post-traumatic causes produces elbow pain, stiffness, and osteophyte formation at the olecranon and coronoid tips that mechanically block terminal extension and flexion. Arthroscopic debridement with osteophyte removal restores motion and relieves impingement symptoms. The Outerbridge-Kashiwagi procedure creates a fenestration in the olecranon fossa from the posterior approach to improve terminal extension.
Elbow osteoarthritis producing pain, stiffness, and mechanical block from osteophytes at terminal motion, managed with arthroscopic debridement or fenestration.
