Hip Posterior Capsular Recess

The posterior hip capsular recess lies between the posterior femoral neck and the posterior hip capsule, and may accumulate fluid in hip joint effusion and posterior hip impingement. It is accessed during posterior hip arthroscopy via the posterolateral portal.

Clinical Notes

Posterior hip capsular synovitis from femoroacetabular impingement produces posterior hip pain with combined hip flexion, adduction and internal rotation (FAIR position). MRI identifies posterior capsular thickening and synovitis. Arthroscopic posterior capsulotomy addresses posterior capsular contracture causing internal rotation deficit in FAI surgery.
