The anterior glenohumeral capsular space lies between the anterior glenoid labrum and the subscapularis tendon, containing the inferior and middle glenohumeral ligaments and the subscapularis recess. It is the territory of the Bankart lesion in anterior shoulder instability.
The anterior capsular space is the primary arthroscopic field in Bankart repair, where the detached labrum is re-approximated to the anterior glenoid rim. Anterior capsular laxity in the HAGL (humeral avulsion of glenohumeral ligament) lesion occurs on the humeral side and is repaired via an anterior portal. The anterior capsular space is injected for glenohumeral joint steroid delivery.
Anterior inferior labrum-ligament complex detachment from the glenoid rim within the anterior capsular space producing recurrent anterior shoulder dislocation, repaired arthroscopically by anchor fixation of the labrum to the anterior glenoid.
