The distal forearm flexor space lies between the flexor tendon layer and the pronator quadratus muscle in the distal forearm, continuous with the carpal tunnel proximally. It is the space where deep flexor tendon lacerations are repaired and where foreign bodies track from wrist wounds.
The distal forearm flexor space is accessed in zone 5 flexor tendon repairs and in exploration of wrist laceration injuries. The pronator quadratus marks the floor of this space. Deep space infection from puncture wounds or cat bites may track through this space into the carpal tunnel and palmar spaces.
Infection tracking through the distal forearm flexor space from wrist puncture wounds producing subfascial forearm swelling with progressive finger flexion restriction, requiring surgical debridement of the space.
