The posterior ankle capsular recess extends between the posterior tibia and the posterior talar dome, accessed via the posterolateral arthroscopic portal. It contains posterior talar osteophytes, loose bodies, and synovitis contributing to posterior ankle impingement.

Posterior ankle arthroscopy via posterolateral and posteromedial portals targets this posterior recess for os trigonum excision, posterior talar osteophyte removal, FHL tendon release, and posterior capsule debridement. Posterior ankle impingement syndrome in ballet dancers and gymnasts involves this recess. The posterolateral portal is placed adjacent to the Achilles tendon with the ankle plantar-flexed.

Synovial proliferation in the posterior ankle capsular recess from posterior impingement or inflammatory arthropathy producing deep posterior ankle pain in plantarflexion, managed by posterior ankle arthroscopy and synovectomy.
