The medial supracondylar ridge is the sharp posterior-medial bony ridge running from the medial epicondyle proximally along the posterior humerus. It provides the proximal origin of the flexor-pronator mass (pronator teres, flexor carpi radialis, palmaris longus, flexor digitorum superficialis, flexor carpi ulnaris) through the common flexor origin. On rare occasions a supracondylar spur projects from this ridge (see ligament of Struthers).
The medial supracondylar ridge is the site of pain in medial epicondylitis (golfer elbow) when proximal involvement extends up the ridge from the common flexor tendon origin. Periostitis of the ridge from repetitive stress produces a fusiform swelling palpable on the medial humerus. A supracondylar process projecting from this ridge (present in 3% of individuals) connected to the medial epicondyle by the ligament of Struthers forms an accessory tunnel that can entrap the median nerve and the brachial artery.
Severe medial epicondylitis from repetitive wrist flexion overload produces pain extending proximally along the medial supracondylar ridge as the flexor-pronator origin inflammation extends to its proximal attachments; treatment is the same as focal epicondylitis with physiotherapy, platelet-rich plasma injection, or surgical debridement.
