Brachialis Humeral Origin Tendon

The brachialis originates via a broad tendinous and muscular attachment from the anterior surface of the humerus — the lower two-thirds of the anterior humeral shaft and the medial and lateral intermuscular septa. The proximal tendinous component blends with the periosteum of the humeral shaft.

Brachialis origin avulsion is a rare injury from forceful elbow flexion deceleration, occasionally occurring in rock climbing falls. The proximal tendinous attachment at the humerus is the site of myositis ossificans following elbow dislocation and anterior elbow trauma — ectopic ossification in the brachialis is the most common location for post-traumatic heterotopic ossification in the elbow. The brachialis origin is the deep structure encountered during the anterior approach to the elbow.

Heterotopic ossification within the brachialis muscle at its humeral origin following elbow dislocation or anterior elbow trauma, producing progressive flexion loss and a firm anterior elbow mass requiring surgical excision after maturation.
