The splenic artery is the largest coeliac branch, supplying the spleen, pancreas, and greater curvature of the stomach via the left gastroepiploic artery. Its tortuous course along the pancreas makes it the artery of reference for distal pancreatic surgery. Splenic artery aneurysms are the most common visceral artery aneurysm (60%), with rupture risk in pregnancy.

Splenic artery aneurysms greater than 2 cm or in women of childbearing age require treatment due to rupture risk — endovascular coil embolisation is first-line; open ligation or splenectomy for unsuitable anatomy. In distal pancreatectomy with splenectomy, the splenic artery is divided at its coeliac origin. Splenic artery pseudoaneurysms from pancreatitis can present as catastrophic GI haemorrhage.
