The posterior medial malleolar artery is a branch of the posterior tibial artery, supplying the posteromedial ankle capsule and the posterior surface of the medial malleolus. It contributes to the malleolar anastomotic network.

Clinical Notes

The posterior medial malleolar artery is encountered during posterior tibial tendon surgery and posteromedial ankle approaches. It provides collateral blood supply to the medial malleolus when the anterior medial malleolar artery is disrupted in fractures. Its disruption may contribute to delayed medial malleolus fracture healing in isolated medial malleolus fractures.
