The peroneal communicating artery connects the peroneal artery to the posterior tibial artery through the posterior intermuscular septum in the distal leg, providing collateral circulation between the two major posterior leg vessels.

Clinical Notes

The peroneal communicating artery is the basis for peroneal-to-posterior tibial collateral flow when either vessel is occluded by peripheral vascular disease or trauma. In diabetic peripheral artery disease where the tibials are preferentially occluded, the peroneal artery via its communicating branch may provide the only continuous perfusion to the foot. Angiography identifies this communicating vessel before planning revascularisation procedures.
