The peroneal anastomotic vein connects the peroneal venous system to the posterior tibial veins in the deep posterior compartment, providing a collateral venous drainage route for the lateral lower leg. It forms part of the deep venous network of the lower leg relevant to DVT propagation.

Clinical Notes

The peroneal anastomotic veins are relevant in compression ultrasound for DVT, where incomplete assessment of the peroneal system may miss isolated peroneal vein thrombosis that can propagate to the popliteal vein. These veins are encountered during fibular free flap harvest, where preservation of the anterior tibial veins ensures leg drainage. Peroneal vein thrombosis presents with lateral calf pain and is managed with anticoagulation.
