The median basilic perforating vein connects the deep brachial veins to the basilic vein at the medial upper arm through the deep fascia, forming part of the cephalic-basilic venous communication system in the arm alongside the median cubital vein at the elbow.
The median basilic perforator is encountered during basilic vein transposition for haemodialysis access, where it must be divided to allow the basilic vein to be lifted to the subcutaneous position. Its division may produce a bruise at the medial arm that resolves spontaneously. In duplex vein mapping for access planning, this perforator is identified to assess the basilic vein diameter and depth.
Division of the median basilic perforating vein during basilic vein transposition for haemodialysis access producing medial arm ecchymosis that resolves within 2 weeks.
