The medial marginal vein drains the medial border of the foot and dorsal forefoot, ascending medially to become the great saphenous vein at the medial malleolus. It is the origin of the great saphenous vein and the most accessible vessel of the medial foot.

Clinical Notes

The medial marginal vein at the medial malleolus is the starting point for great saphenous vein stripping procedures and endovenous ablation. It is the traditional point of access for great saphenous vein cutdown in emergency venous access. In diabetic foot disease, medial foot venous congestion from medial marginal vein thrombophlebitis contributes to tissue swelling.

Superficial vein thrombosis in the medial marginal vein producing medial foot swelling and a palpable cord, managed with compression and anti-inflammatory medication.
