The inferior gluteal artery exits below the piriformis to supply the inferior gluteus maximus and provides the pedicle for the inferior gluteal artery perforator (IGAP) flap and the posterior thigh flap. It also provides small articular branches to the hip capsule. The inferior gluteal artery perforator flap uses lower buttock skin for breast reconstruction with a longer pedicle than the SGAP flap.

Hip arthroplasty approaches through the posterior capsule must be designed to avoid interrupting both the inferior and superior gluteal arteries. The transgluteal approach (Hardinge lateral approach) is safer in this regard than the posterior approach. Inferior gluteal artery injury in femoral fractures produces haematoma in the posterior thigh.
