The descending geniculate vein accompanies the descending geniculate artery from the distal femoral artery, draining the anteromedial knee and superomedial capsule into the femoral vein via the saphenous vein or directly. It forms part of the medial knee venous drainage.
The descending geniculate vein is relevant in planning medial thigh perforator flaps and in medial knee dissection for MPFL reconstruction. Its accompaniment of the descending geniculate artery makes it a guide to the artery during dissection. Post-thrombotic changes in this vein may contribute to medial knee swelling after knee surgery in predisposed patients.
