The ascending genicular vein accompanies the descending genicular artery in reverse, draining the medial knee and distal medial thigh into the femoral vein at the adductor hiatus level. It is the venous companion of the medial knee arterial supply.
The ascending genicular vein is encountered during medial approach to the distal femur and in medial condyle tumour resection. It contributes to the venous drainage of the medial knee compartment in total knee arthroplasty and is the venous pedicle companion in medial knee perforator flap surgery.
Bleeding from the ascending genicular vein during distal femoral plating or medial condyle fracture fixation producing medial thigh haematoma requiring intraoperative haemostasis.
