Small perforating branches from the lateral inferior geniculate artery penetrate the anterolateral knee capsule and fat pad, providing the microvascular supply to the anterolateral capsular structures including the anterolateral ligament.

Clinical Relevance

Clinical Notes

The anterolateral knee perforating vessels supply the anterolateral ligament (ALL) and anterolateral capsule, which are increasingly recognised as important structures in rotatory knee instability. Damage to these vessels in anterolateral knee injury contributes to anterolateral capsule healing impairment. ALL reconstruction preserves the vascular bed for graft incorporation.

Disruption of anterolateral perforating vessels in high-energy lateral knee trauma producing anterolateral capsule ischaemia that impairs healing of the anterolateral ligament and contributes to rotatory instability.
