The zona orbicularis is the circular collar of the deep hip capsule formed by the circular (spiralling) fibres of the three primary hip ligaments creating a ring around the femoral neck. It holds the femoral head in the acetabulum by suction and pressure, preventing distraction. It becomes visible during hip arthroscopy as the circular fibres at the base of the femoral neck.
Prevents distraction of the femoral head from the acetabulum; provides synovial fluid circulation to the femoral head articular cartilage
The zona orbicularis is significant in hip arthroscopy as it forms the inferior boundary of the peripheral compartment. During femoral neck osteoplasty for femoroacetabular impingement (FAI), the zona orbicularis level marks the distal limit of bone removal. Over-resection distal to the zona orbicularis risks femoral neck fracture.
Zona orbicularis serves as the distal limit for femoral neck osteoplasty in FAI surgery — bone resection distal to this level risks femoral neck fracture.
