The TFL tendon blends into the iliotibial band rather than inserting directly on bone, making TFL the primary muscular tension source for the IT band. This arrangement allows TFL to transmit force from the anterior iliac crest to the lateral knee through the IT band. TFL tightness assessed by the Ober test reflects the combined tightness of the muscle-tendon unit and IT band.

TFL tightness is a key factor in IT band syndrome, lateral hip pain, and patellofemoral syndrome. Foam rolling the TFL-IT band complex reduces tissue tension. Progressive hip abductor and external rotator strengthening reduces TFL compensatory overactivity in runners.
