Tensor Veli Palatini Tendon at Hamulus

The tensor veli palatini muscle winds around the hamulus of the medial pterygoid plate via a fibrocartilaginous pulley, and its tendon then spreads into the soft palate as the palatine aponeurosis. This unique tendon-around-a-bony-pulley arrangement allows the muscle to tense the soft palate.

The tensor veli palatini tendon at the hamulus is the target in endoscopic eustachian tube dilation, where tensor dysfunction causes eustachian tube obstruction. Cleft palate repair must reconstruct the tensor tendon's palatine aponeurosis contribution for normal palatal function and speech. The hamulus fracture (rare) disrupts this pulley mechanism, causing tensor muscle dysfunction.
