Extensor Retinaculum (Ankle Superior)

retinaculum musculorum extensorum superius pedis

The superior extensor retinaculum is the proximal transverse band above the ankle joint constraining the four anterior compartment tendons. Anterior ankle impingement from osteophytes can compress the tendons and the deep peroneal nerve between the osteophytes and the retinaculum. Extensor tenosynovitis under this retinaculum produces dorsal ankle pain.

Region: Foot & Ankle
Biomechanics

Function & Actions

Maintains the extensor tendons against the ankle during dorsiflexion; prevents bowstringing of the extensor tendons

Clinical Relevance

Clinical Notes

The superior extensor retinaculum is occasionally released in anterior ankle impingement syndrome when it contributes to soft tissue compression against the anterior tibiotalar osteophytes. Extensor retinaculum release decompresses the deep peroneal nerve in cases of anterior tarsal tunnel syndrome.

Pathology

Common Injuries & Conditions

Anterior Tarsal Tunnel Syndrome

Deep peroneal nerve compression under the superior extensor retinaculum producing dorsal foot numbness and first web space pain managed with retinaculum release.
