The SCM inserts on the mastoid process, producing the characteristic posture of congenital muscular torticollis: ipsilateral head tilt with contralateral rotation. The SCM fibrosis in congenital torticollis produces a palpable mass in the neonatal period that may resolve with stretching or require surgical release. The SCM is sectioned at its sternal and clavicular origins (or at its mastoid insertion) for torticollis correction.

Clinical Relevance

Clinical Notes

Congenital muscular torticollis from SCM fibrosis (from birth trauma or intrauterine malposition) produces the classic head tilt with contralateral chin turn. Physiotherapy stretching resolves 85% of cases by 12 months. Surgical SCM release (bipolar release — origin and insertion) is required for residual torticollis after 12-18 months of stretching.
