The rhomboid minor inserts via a thin tendinous sheet onto the medial scapular border at the level of the scapular spine, at the triangular smooth surface between the spine and the superior angle. Its insertion is superior and separate from the rhomboid major insertion, providing scapular retraction at the superior border.
Rhomboid minor tendinopathy at the scapular spine insertion produces focal interscapular pain at the superior medial angle. It is less commonly involved than rhomboid major but contributes to the upper interscapular pain syndrome. Differentiated from levator scapulae insertion pain by its more medial and inferior location relative to the superior angle. Targeted injection at the scapular spine level addresses this tendon specifically.
Focal pain at the superior medial scapular border from rhomboid minor insertion degeneration, producing upper interscapular aching with resisted scapular retraction and elevation.
