The peroneus tertius is an inconstant muscle (absent in 8 to 10 percent of people) whose tendon, when present, runs through the fourth compartment with the EDL to the fifth metatarsal dorsum. Its avulsion produces a fifth metatarsal dorsal avulsion fracture distinct from the styloid avulsion of the peroneus brevis. Its absence causes no functional deficit.

Peroneus tertius tendon avulsion produces dorsal fifth metatarsal base pain that must be distinguished from Jones fracture at the diaphyseal-metaphyseal junction and peroneus brevis styloid avulsion. CT confirms the fracture location and cortical integrity. Most peroneus tertius avulsions heal conservatively.
