The IT band inserts on Gerdy's tubercle of the anterolateral tibial condyle, completing the lateral knee stabilising system alongside the LCL and popliteus. During running, the IT band moves anterior to the lateral femoral epicondyle in extension and posterior in flexion — the repetitive friction at the 30-degree impingement zone of flexion-extension produces IT band syndrome.

Clinical Notes

IT band syndrome is the most common running injury, producing lateral knee pain specifically at 30 degrees of knee flexion that worsens progressively with running. Ober's test assesses IT band flexibility. Foam rolling, hip strengthening, and running technique correction (reducing contralateral pelvic drop) are first-line management. Surgical IT band Z-lengthening for refractory cases is rarely needed.
