Peroneal Retinaculum (Inferior)

The inferior peroneal retinaculum is the distal restraint for the peroneal tendons as they course along the lateral calcaneus, arising from the peroneal tubercle. It divides the two peroneal tendons — the brevis runs superior (closer to the bone), the longus inferior (farther from the bone). The peroneal tubercle between the two tendons is a site of peroneal tendon pain from hypertrophic tubercle impingement.

Clinical Relevance

Clinical Notes

A hypertrophic peroneal tubercle can impinge between the peroneal longus and brevis tendons at the inferior retinaculum level, producing lateral hindfoot pain during heel inversion. MRI shows a prominent peroneal tubercle with adjacent tenosynovitis. Surgical shaving of the hypertrophic tubercle resolves impingement.
