The iliocostalis cervicis is the cervical component of the iliocostalis column, arising from the upper ribs and inserting onto the posterior cervical transverse processes. Its tendinous origin from the ribs is in the thoracic region but its action on the cervical spine makes it clinically relevant in lower back-to-neck continuity.

The iliocostalis cervicis tendon at the rib origins is a source of lower thoracic and upper lumbar trigger points that refer pain to the cervical and upper thoracic spine. Its continuity with the lumbar iliocostalis explains the long kinetic chain from lumbar dysfunction to cervical pain patterns seen in postural syndromes.
