The extensor tendons of the foot pass under the superior and inferior extensor retinacula at the ankle in separate compartments before fanning across the dorsal foot to the toe extensor expansions. They are responsible for lifting the toes during swing phase of gait and for decelerating plantarflexion eccentrically during heel contact. Extensor tendon tenosynovitis from shoe pressure or overuse produces dorsal foot and ankle pain reproduced by resisted extension.

Dorsal foot ganglion cysts arising from the tarsometatarsal joint capsule are frequently confused with extensor tendon tenosynovitis — both produce dorsal foot swelling but ganglion is transilluminatable and firm while tenosynovitis is more diffuse. The extensor retinaculum constrains the extensor tendons and its entrapment of the deep peroneal nerve and anterior tibial vessels causes anterior tarsal tunnel syndrome.

Dorsal ankle and foot tendon sheath inflammation from shoe pressure or overuse producing pain reproduced by resisted toe extension, managed with activity modification and padding before tendon sheath injection.
