The four EDC tendons pass through the fourth extensor compartment beneath the extensor retinaculum at the wrist, diverging to each finger as they cross the radiocarpal joint. At the wrist level they are accessible for repair after dorsal wrist lacerations and tendon transfer harvest.
EDC wrist-level lacerations in zone 5 require repair of individual tendon slips under magnification to restore independent finger extension. The juncturae tendinum (tendinous interconnections between the EDC tendons on the dorsal hand) distal to the wrist allow some extension of adjacent fingers even with single tendon laceration. Zone 5 is the most commonly injured extensor zone from dorsal wrist lacerations.
