The ECRB passes through the second extensor compartment alongside the ECRL and inserts centrally on the third metacarpal base, producing pure wrist extension without significant radial or ulnar deviation. Its central position makes it the most efficient wrist extensor for power grip, as wrist extension during grip stabilises the wrist in the power position. At its lateral epicondyle origin it is the primary pathological tendon in lateral epicondylalgia.

Clinical Notes

The ECRB is the primary working tendon for wrist extension during forceful gripping activities such as tennis backhand, manual screwdriving, and keyboard use, explaining why these activities exacerbate lateral epicondylalgia. Extensor digitorum exercises in the eccentric lengthening position specifically load the ECRB origin for rehabilitation.
