The distal triceps tendon inserts onto the posterior olecranon via a complex footprint with central and lateral components. The central portion inserts into the olecranon tip and the lateral expansion blends with the anconeus and the forearm fascia. This complex insertion is important in surgical repair technique.

Clinical Notes

Distal triceps tendon rupture repair requires reattachment of the central and lateral tendon components to the olecranon footprint to restore the normal insertion anatomy. Transosseous tunnels or suture anchors placed at the correct olecranon footprint location prevent repair failure. The lateral expansion to the forearm fascia (the triceps retinaculum) is also repaired to restore the complete extensor mechanism.

Distal Triceps Tendon Rupture at Olecranon

Complete distal triceps tendon avulsion from the olecranon from forced elbow flexion during extension, producing loss of active elbow extension and a palpable gap at the posterior elbow, repaired by transosseous or anchor fixation to the olecranon footprint.
