Abductor Hallucis Longus Tendon

tendo musculi abductoris hallucis longi (i.e. abductor hallucis)

The abductor hallucis tendon is the principal intrinsic tendon of the great toe, running from the medial calcaneus along the medial plantar arch to the medial sesamoid and proximal phalangeal base. Its weakening in hallux valgus allows the great toe to deviate laterally as the abductor force decreases relative to the adductor. Strengthening the abductor hallucis through short-foot exercises is a cornerstone of conservative hallux valgus management.

Region: Foot & Ankle
Biomechanics

Function & Actions

Great toe abduction and medial MTP stabilisation; medial plantar arch dynamic support

Clinical Relevance

Clinical Notes

The abductor hallucis is the primary target of intrinsic foot muscle strengthening for plantar fasciitis, hallux valgus, and overpronation. The short-foot exercise (doming the foot by drawing the metatarsal heads toward the heel without toe curling) specifically activates the abductor hallucis and intrinsic arch muscles.

Pathology

Common Injuries & Conditions

Abductor Hallucis Weakness

Progressive medial arch insufficiency and hallux valgus from abductor hallucis atrophy managed with short-foot exercise and medial arch orthotic support.
