The radial proper digital nerve of the thumb is a terminal branch of the median nerve supplying the radial (lateral) surface of the thumb pulp. It is the primary sensory nerve for pinch grip and is critical for two-point discrimination at the thumb tip.

Clinical Notes

The radial digital nerve of the thumb is the most important sensory nerve in the hand for precision grip and is the first priority in digital nerve repair after thumb injury. It runs superficially beneath the skin at the radial border of the thumb and is injured in gamekeeper thumb avulsion, A1 pulley injections, and carpometacarpal arthroplasty. Loss produces an impaired pinch sensation described as 'dead thumb.'
